An abridged interpretation
Dr Olof Lagerlöf , born 1916, is an practicing eye specialist. He commented on the relationship between vitamin B12 deficiency and toxic blindness in Läkartidningen 2000;97:25. In the Swedish edition of The Rondel 2000;4 (Rondellen, title "Recension"), he gave a broad overview of his experience as an ophtalmologist.
One main message of Dr Lagerlöf was that the art of perimetry, like other sorts of physical examination and history-taking, is dependent on the skill and experience of the examiner. "This finger, digitus secundus, is by experience as sensitive an index of defects in the visual field as the most sophisticated equipment!"
Two basic methods of clinical perimetry are confrontation and campimetry. Among the clinical signs commented by Dr Lagerlöf are lesions in the loop of von Willebrand, often due to meningeoma in the anterior part of the sphenoid bone, and the migraine scotoma, typical for lesions in fissura calcarina.
Dr. Lagerlöf emphasized that examination of visual acuity and intra-ocular pressure should be combined with perimetry in order to improve patient safety.
